The format and fees are as follows: Schedule in advance a phone appointment by calling 1-480-699-4867. First 15 minutes: $40.00 Each Additional 15 minutes: $20.00 You can make your initial payment online of $40.00 at www.myrehab.com/pay . Fee includes the consultation, discussion of your past care, identifying locally available resources and seeking out healthcare providers in your area that utilize treatments in line with the DSR approach. We do not treat or prescribe interventions over the phone. Professional Consultations for Patients Matt will be glad to discuss patients with other professionals. Please call to schedule a phone appointment. His experience is that he often learns as much in return during such exchanges. Consequently, all consultations under 15 minutes are professional courtesy. If during the conversation it becomes apparent more time will be needed, that matter will be discussed at that time.
